💨 Abstract

On February 12, 2022, a 14-year-old girl, Ruth Szymankiewicz, managed to self-harm and later died due to inadequate supervision at Huntercombe Hospital in Berkshire. The childrens' mental health ward was severely understaffed, with at least half the staff missing. Ruth, who was under strict observation for an eating disorder, was left unsupervised by an agency worker using a false name and without proper induction.
