The Titan submersible disaster, which killed five people in June 2023, was allegedly facilitated by OceanGate's use of intimidation tactics to avoid regulatory scrutiny. The US Coast Guard's report identified eight primary factors leading to the implosion, including a toxic workplace environment and misrepresentation of safety. The sub's carbon fiber construction, deemed inappropriate for deep-sea use, and lack of proper certification contributed to the tragedy.
