Arizona's mental health director alleges that $60 million in fraud has been flagged - involving phantom billing for Medicaid services. The heads of a behavior health company, church and 20 other individuals were named in the latest indictment involving a state-funded mental health fraud. The alleged fraud involved improper billing, payment from unlicensed sober homes and money laundering.
